Any household that can afford that many monkeys can probably afford to have a family doctor.
If I want to call MY doctor, a specific person, then no I can’t.
If I want A doctor, then yes, I can just call. Any time 24/7, I can talk to someone. Worst case, it’s a nurse.
And I don’t have to be rich, it’s free.
The phrase “snitches get stitches” makes a lot of assumptions about the availability and expediency of healthcare.
In the US we say, snitches better get that thing looked at before it gets any worse.
Snitches without health insurance get butterfly bandages and some nasty scarring.

Doctors used to and still do make house calls. You just have to be wealthy
concierge doctors.
In Germany it still is.
There’s a thing called “concierge doctor.” You pay $X per month and get basically 27/7 access to your doctor, and free consultations. It’s not covered by insurance, and specialists (which is almost everything) still require insurance or other payment.
If you can afford it, concierge is absolutely fantastic.
27/7 is quite a lot
It’s space. There’s actually 28 hours in space, but she does get bathroom breaks.
Rural Town back when that rhyme was coined? Sure. He was just over of your neighbor’s of you were even lucky enough to have a doctor in town. But not in an urban out semi urban area.
My dad grew up in NYC and when he was a kid in the 50s a doctor saved his life by coming to their apartment in the middle of the night. My dad’s family were super poor and couldn’t pay the doctor, so I guess it really depends on the kindness of the doctor.

My family doctor made a house call in 1978, that’s the last time I heard of it happening. Though he probably continued until he retired in the early 80s.
