My impression is that that is not just any random paparazzi. He’s either a paparazzi who got tipped off by Mako, or that he’s a detective of sorts, hired by Mako to take compromising photos of them.
In fact, I’m surprised that there are no hidden camera indoors.

Yeah, this wasn’t very clear in the manga, but I think the anime makes it more or less explicit that she tipped off the paparazzi. Kana texts her exactly where they are going and she ducks away and makes a call right after being told they are heading to his studio.

I didn’t even think of that! But it really makes sense. Why would she do that though? Out of spite? Well she wasn’t exactly friendly but this is just malicious.
