There are neck guards, which most players don’t wear. Much of the protective equipment is by choice, and leagues need to remove the choice

They’re already covered in protection everywhere you could put it, you can’t put anything on the neck because then you can’t turn it and the strangulation risk gets added. This is just a freak accident like many sports have occasionally. Football players sometimes suffer cardiac arrest when tackled, baseball players get hit in the chest and have the same, or get knocked in the head, sports all have some level of danger

Neck guards exist in hockey to prevent this injury. They are mandatory in most places in minor hockey but become optional in adult leagues.
