A cars cellular service is managed by the manufacturer. It’s not a traditional plan like you have for your phone. The manufacturer typically leverages the cellular service to sell you additional features. The base functionality for their benefit is always there sending data about the car. It’s also how over the air firmware updates are applied to the infotainment system.

But how does it work? Does the car have a mobile data plan included? Or is doing it through connected smartphone?

It doesn’t need a data plan like the ones you or I buy. There are a lot of services that buy bulk open data services from telcos. And they’ll just keep doing so as long as the modem is still supported.
My CPAP has a 2G cellular modem in it and it was sending my usage data to my doc (and probably others) every morning for years until the last 2G supporting cell tower in my area was shut down.

The car indeed has mobile data. A Tesla has a permanent 4g connection.
