Consideration of accessibility at the design stage should not require any "someone else", because it is a default collective mentality.
As an example cited elsewhere, the decision to use some "fancy slider" demonstrates the collective problem: _no-one_ is (apparently?) available at the _design_ stage to even think deeply, conceptively about the process which led to "fancy slider" being agreed to be a good a idea. No-one is at individual fault, it is systemic which may be due to "group think". Either widen the pool or deepen the awareness, to minimise such problems.
