Context: In 1982 a Soviet submarine crew was running a training exercise. They were supposed to simulate a missile launch but the real thing went flying. The launch key wasn’t even in the right position, but the missile didn’t care. It shot out of the sub and landed in the ocean. The crew was absolutely stunned. The incident got the nickname ‘The Whoopsie Missile’ for a reason.
