Reflections have existed, presumably, longer than humans and I don’t know abiut you, but I can use a reflection to see my butthole and I still sometimes get hemorrhoids. 🤷♂️

Hemorrhoids are caused by excessive pressure in the veins around the anus and not because they just like to come out when you’re not looking lol.
Besides, people walk around with all kinds of visible swelling/irritations on other body parts all the time when they can’t immediately treat them.

Maybe OP is describing a prolasped rectum, where some intestine is pushed out, rather than a hemorrhoid like you describe.
