Interesting reasoning. I think they could have made it optional, but I suppose they’re balancing the whole game around it, so maybe it would have made a non-permadeath playthrough too easy/unbalanced.

That’s the right approach, for sure. My wife and I watched it, me having played the game, and her having tangentially seen me playing bits and pieces but basically knowing nothing about the plot. It was interesting comparing her reactions with mine, but I think I got more out of the show being able to compare to my memory of the game.

The part of the urethra that passes through the prostate is usually bent. As I said, I’m only guessing, but I think by angling the penis down and pushing the prostate forward from behind, it should make it less of a sharp angle for the scope to pass through, therefore reducing pain and risk of urethral injury.
