I think Cryptpad has a good forms builder. Try it here (official instance)
Baserow and n8n both have form builders built-in, so if you’re using one of these tools already it might be worth checking out.
Lime Survey. I needed a self hosted solution and after a long search and evaluating a lot of alternatives, finally settled on this because a lot of advanced features were needed.
Pros: all possible features that you could need and a lot more. Admin interface is very powerful. Easy to install, extend. Cons: PHP. Complicated (as it’ has a lot of options). Scripting language is a pain in the butt.
There is Ohmyform which is great but not updated at this point.
Budibase.

If you already have Nextcloud running you can use the Nextcloud Forms app

It’s really basic, I wouldn’t recommend it as a form builder, more like a nice added feature.
